Change in Physicals this year:
Pre-participation Physical Examinations completed for the 2019-20 school year will be accepted during the 2020-21 school year upon submission of a parent and athlete signed waiver (previously known as Form B).
Students who did not have a Pre-participation Physical Examinations completed for the 2019-20 school year must complete a Pre-participation Physical Examinations completed for the 2020-21 school year.
In other words, if your son/daughter played 7-12 sports last year, they would not need a physical this year, you would just need to fill out the waiver. Students in 7th grade and any students that did NOT participate in 7-12 sports last year will need a physical. Appropriate forms are available in the office.
